
The contract
From 2022 until the contract closed on May 31, 2026, Shire Scientific served Hennepin County residents living with serious and persistent mental illness, under contract with Hennepin County Human Services and Public Health.
The purpose of Adult Mental Health Targeted Case Management is simple to say and hard to do: keep people well and at home, and out of hospitals and treatment centers whenever that is possible.
At our peak we served about 200 residents at a time.

What our case managers did
Functional Assessment. For every resident we assessed all 14 life domains required by Minnesota law, recording strengths as well as impairments, and renewed the assessment at least every 12 months.
Individual Community Support Plan. The resident and the case manager set goals together and reviewed them at least every six months.
Referral and linkage. We connected residents to mental health care, health insurance, disability verification, housing and employment, and helped keep Medical Assistance active.
Coordination and monitoring. We stayed in contact with each resident every month and followed Minnesota Health Care Programs guidance.

The team and how we worked
Our case managers held degrees in social work, public health, psychology, sociology and health and human service administration. They were supervised by a Licensed Psychologist, a Licensed Independent Clinical Social Worker and a Licensed Professional Clinical Counselor.
A case manager was assigned within five business days of each county referral. No case manager served more than 24 to 28 residents a month.
We met residents at our office, in their homes, in community settings such as public libraries, and by interactive video. We served residents in English, Spanish and Somali.
In our proposal to the county we wrote that equity is not the organization's plan, it is the organization's DNA.

Enrolled provider with Minnesota Health Care Programs, with an agency National Provider Identifier.
In-network with all five managed care organizations in Minnesota's Prepaid Medical Assistance Program: Hennepin Health, HealthPartners, UCare, Medica and Blue Cross and Blue Shield of Minnesota.
Full insurance coverage, including professional, general, automobile and cyber liability.
Written policies for data privacy, client grievances, quality improvement and staff education, and a HIPAA-aligned electronic records system.
